Registered Nurse (RN) Job Description
Unique position available for a motivated and flexible RN working with the Iraqi refugee population at a busy Adult Day Health Care. Daily hours are 8:30am-3:00pm, M-F. Must be a team player and willing to participate in many different services and tasks. Computer skills a must. Qualifications: · Must be licensed by the California Board of Registered Nursing. · The RN shall be knowledgeable of the physical, mental and social needs of the elderly. · The RN shall have experience in community based public health programs or clinics providing services to the frail elderly. · Must possess current CPR and First Aid certification in addition to a TB clearance and health examination from a physician (at time of hire or within 7 days of hire). Must be a team player to ensure close, clear and timely communication with a multidisciplinary team. Must possess intermediate computer skills and be comfortable with electronic records keeping and updating. Specific Duties: · Initial and continuing assessment of each participant in the program by the RN in the multidisciplinary team and development of a written plan or care. The RN shall assess each participant to determine needs for personal care and assistance in activities of daily living such as bathing, grooming, toileting, and eating; include specify long and short team goals for each participant. · Provision of restorative and supportive nursing as indicated by the participants’ treatment plan. · Administration of medications or monitoring of self-administration of medication by the participants. The RN shall monitor, administer, and record prescribed medications. · Supervision of personal care given by Program aide. The RN shall supervise the activities of the program aides when the latter provide personal care to the participants. · Maintenance of participant’s health record which includes: a) Signed quarterly progress notes from each member of the multi-disciplinary team and outside consultants. b) Records of medications and treatments administered. c) Quarterly narrative nursing notes, or more frequently if necessary, indicating participants’ progress towards achieving health goals. · RN shall assess any urgent participant care, including arrangement for transportation. The nurse shall assess any urgent participant care situations to determine the appropriate course of action, provide immediate emergency care and arrange for transport to obtain medical care. · Notification of the Physician of any marked change in the participant’s condition. Coordination of the treatment plan with participant care physician. The RN shall be the liaison between the Center and the participants’ personal care physician. · Supervision of a maintenance program designed by an occupational and/or physical Therapist. The RN shall be trained by the Occupational and/or Physical Therapist in the specific of the maintenance program. The RN shall supervise the aides or other staff assigned to carry out specific activities developed by occupational or physical therapist to help the participant maintain current levels of functioning. · Provide observation and monitoring of health status. · Provide supportive and restorative nursing as indicated by treatment plan. · Provide assistance to the participant in obtaining medical services from providers other than adult day health staff. · Provide health education and counseling to the participant and family when indicated by the plan of care. · Training in self-administration of medications shall be provided to all participants based on the recommendation of the multidisciplinary team.
