We are currently seeking full-time and part-time RN’s to be part of our amazing team of clinicians. Responsibilities: · Completes OASIS assessments to determine skilled needs of each patient. · Develops Plan of Care to be submitted for provider approval. · Provides continuous assessments and updates Plan of Care accordingly. · Assesses goals at each skilled visit to determine the need for further care. · Provides teaching to family and patients to prevent rehospitalization and increase knowledge of self-care. · Provides all treatments and care per providers orders. · Evaluates patients need for other services such as physical therapy, occupational therapy, speech therapy, and home health aide services. · Communicates daily and as needed with interdisciplinary team, family, and medical provider regarding patient care. · Coordinates community services for patients as needed for safe discharge plan. · Responsible for managing and scheduling a caseload of patients in coordination with all nursing staff. · Attend training and staff meetings as scheduled. · Completes online training as required. · Tracks mileage for reimbursement purposes · Responsible for completing all admission and discharge paperwork accurately and returning it to office in a timely manner. · Takes part in weekend and holiday rotation. · Demonstrates good teamwork, work ethic, and written/verbal communication skills; able to adapt to change easily. · Performs supervisory visits of Home Health Aide and LPN staff. · Observes all agency policies and procedures. · Assists with office duties, as scheduled by the Clinical Manager Job Types: Full-time, Part-time Standard shift: Day shift Evening shift Night shift Weekly schedule: Monday to Friday Work Location: One location
