FLSA Status: Non-exempt JOB TITLE: LICENSED PRACTICAL NURSE Department: Nursing Reports To: Nursing Supervisor and/or Charge Nurse This job description, defining qualifications, primary (essential) and secondary (non-essential) functions, responsibilities and duties, is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. ESSENTIAL FUNCTIONS: Management Function Requirements: Supervises and evaluates all direct resident care and initiates corrective action as necessary. Provides clinical supervision of nursing assistants providing direct resident care. Follows staffing guidelines and makes nursing assignments according to qualifications of staff and characteristics of the resident population. Assumes responsibility for compliance with federal, state and local regulations within the assigned unit. Counsels nursing assistants and recommends disciplinary action, when necessary, to the Director of Nursing Service. Instructs nursing assistants in proper, preventive safety measures and use of adaptive equipment to meet the needs of each resident. Controls supplies and equipment within the unit. Collaborates with interdisciplinary team to ensure that high quality resident care is maintained. Prepares nursing assistants' assignments and explains all pertinent resident care. Supervises the application and removal of all postural supports and restraints. Supervises distribution of nourishments. Orders, receives and documents medication received according to facility procedure. Licensed Practical Nurse Page 2 of 9 Supervises response to residents' calls for assistance and responds to call lights as necessary. Insures that all physician renewal orders and physician documentation is timely and appropriate. Plans and conducts unit staff meetings as needed and records problems and recommends action. Suggestions for change are referred to appropriate person. Patient Care Requirements: Obtains report from nurse he/she is relieving and records sufficient information to implement appropriate follow-up action as necessary. Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary. Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action. Identifies resident problems and emergency situations, and initiates immediate "life saving" measures within the scope of nursing practice and in accordance with advance directives. Administers and documents medications, enteral nutrition, and treatments per the physician's order and accurately records all care provided and observations daily. Performs comprehensive assessment of each resident. Documents comprehensive assessment of each resident with review by Registered Nurse. Develops an accurate comprehensive care plan based on each resident's comprehensive assessment. Assesses on a weekly basis the skin condition, stage, size (measurements), site, depth, color, drainage and odor of condition. Obtains physician's orders and any revisions for all treatments and medications needed. Renders immediate assistance to any resident in need. Provides intravenous therapy according to facility policy and procedures. Provides respiratory care according to facility policy and procedures. Assists physician during treatments and examination of residents. Checks meal trays to assure proper diet is served and residents are assisted as necessary. Observes, evaluates and documents all changes in residents conditions.
