Registered Nurse (RN) Job Description
JOB DESCRIPTION: The field staff Registered Nurse (RN), under the direction of and in accordance with the physician's orders, administers professional skilled nursing care services to patients in their homes. RESPONSIBILITIES: The Registered Nurse's duties shall include but shall not be limited to the following: Makes initial and ongoing comprehensive assessments and plan of treatment of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points. Advises the Home Health Agency and Nursing Manager/Supervisor of Treatment Plan after initial evaluation is completed. Submits initial evaluation and documentation to the home health office as soon as documents are completed but no later than seven (7) days. Makes follow-up visits to patients as assigned by the Nursing Supervisor Regularly re-evaluates the patient's progress and adjusts the care plan accordingly, and reports to office and nursing manager/supervisor Provides services that require substantial specialized nursing skills and initiates appropriate preventive and rehabilitative nursing procedures Promotes health and diseases prevention Informs the physician and appropriate personnel of changes in the patient's condition and needs as needed Assists in the coordination of all home health services provided Counsels and instructs the patient and family in meeting nursing and related needs Promotes client/family independence Prepares clinical and progress notes and appropriate communications for all follow-up visits Refers patients to other services as needed Assigns, supervises and teaches other LVNs/CHHAs Administers medication according to the California Nurse Practice Act and State regulations and physician orders Supervises Certified Home health Aides at least every two weeks in the patient's home Plans for discharge of patients from home health services Evaluates outcomes of care Meets all agency policies relating to documentation Participates in required in-service training programs Participates in team conferences Participate in the agency’s QAPI program Submits completed clinical and visit reports to the office every Monday (or first day of the work week if Monday is a holiday) no later than 12:00 noon Subject to “on-call” status on a rotational basis, as the agency is required to have at least one RN “on-call” 24 hours a day.
