Home Health Aide (HHA) Job Description
JOB DESCRIPTION:
The Certified Home Health Aide (CHHA) administers personal care and service to patients under supervision of the nurse, physical therapist, occupational therapist or speech pathologist.
RESPONSIBILITIES:
Follows treatment plan as outlined to provide patient care under the supervision of the Case Manager or the Registered Nurse
Observes patient and notifies the nurse in charge or nursing supervisor of significant changes in the patient's condition, physical or mental
Takes and records vital signs. (i.e. Temperature, pulse, and respiration)
Cares for patients served including the physical, emotional and developmental needs
Assist patient with personal care:
a. Helps patient into tub or shower
b. Bathes patient: Bed bath, Sponge tub or shower bath, Sink, tub or bed shampoo
c. Toileting and elimination
d. Grooming and shampooing of hair
e. Shaving with electric or safety razor
f. Oral hygiene
g. Foot care
h. Skin care
i. Nail care
• Assists patient in ambulation:
a. in or out of bed
b. With prescribed exercise
c. Dressing and undressing
d. Into chair, wheelchair, walker, walking or in passive exercise, as instructed by Physical Therapist, Occupational Therapist, Registered Nurse or approval of the physician
Assists patient in normal range of motion as instructed by Physical Therapist, Occupational Therapist, Registered Nurse or approval of the physician
Prepares meals, therapeutic diets and fluid intake as required
Encourages patient to become increasingly self-reliant as the recovery progresses, by encouraging and assisting patient to do more of his own personal care duties, consistent with prescribed treatment
Shops for patient's needs in small quantities, if there is a store nearby
Overseas patients in following prescribed treatment such as oral medications that are ordinarily self-administered
Cleans equipment according to instructions from nurse
Does personal laundry for the patient
Maintains cleanliness, safety, and healthfulness of sickroom and immediate environment of patient:
a. Performs light housekeeping duties in room and surroundings
b. Changes bedding with or without patient in bed
Recognizes emergencies and has knowledge of emergency procedures
Reminds the patient of medical and dental appointments
Records patient activity and service performed in medical record
May do simple dressing changes, if instructed by the Registered Nurse
Performs household services which will facilitate the patient's self-care at home and which are necessary to prevent or postpone institutionalization
Meets all agency policies relating to documentation
Attends and participates in team conference
Attends and participates in ALL in-service education
Participate in the agency’s QAPI program
Submits to the office weekly completed clinical notes, route sheets and other visit report forms
