Registered Nurse License for the state of Maryland Current BLS for Healthcare Providers Prior experience in home care preferred, positive attitude a must! 2-3 years as a Registered Nurse 2-3 years acute care setting experience 2-3 years Home Care setting experience Vital Sign Home Care, licensed home care provider in the state of Maryland serving Anne Arundel County, Baltimore City, Baltimore County, Harford County and Howard County areas. Position Summary: Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians. Essential Functions: Completes the admission process appropriately for care services per regulatory standards and per the policies and procedures of Vital Sign Home Care. Completes thorough and accurate Clear Care data and plan of care. Performs initial and ongoing assessment of physical, psychosocial and environmental needs of patients in the home setting. Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional care services. Provides supervision for Certified Nursing Assistant as appropriate per the policies of Vital Sign Home Care and the Maryland regulatory agency.
