A licensed Registered Nurse with at least one year of experience in Acute Care/ Rehabilitation Nursing or Home Health Care. Critical thinker, problem solver and motivated clinician who likes to work independently and communicates with patients, family/caregivers, physicians, and office staff well. RN will be responsible for evaluating and treating patients in the comfort of their residence and follow/delegate & monitor established plan of care to ensure desired outcomes.
