Hospice RN Care Manager oversees 12-15 patients and coordinates with the IDG team to ensure continuity of care. We provide personalized care to our patients, ensuring comfort and peace of mind to the patient and their family. The Hospice RN Care Manager oversees the hospice patient's care and will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. RN case managers evaluate a patient's physical condition and develop care plans that address any of the patient's medical needs or needs for social and spiritual services. Apart from providing comfort to patients, RN case managers are adept at counseling and guiding patient's families as well. Minimum Qualifications: Graduate of an accredited nursing program. Current California Registered Nurse license. At least one (1) year of hospice or palliative care experience Minimum of two (2) years of experience as a professional nurse within the last five (5) years. Experience in a case management role Expertise in pain management, symptom control, crisis intervention, and family support. Excellent demonstrated critical thinking skills and superior clinical judgment. Excellent computer skills. Excellent customer service skills. Reliable cell phone with local number and coverage. Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable personal transportation. Ability to work independently and manage time effectively. Ability to make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting.
