The Clinical Manager function is critical in today’s home health care and hospice organization. This person plays a key role within his or her agency to support not only patient care, but also essential business operations. This role is involved with both the financial and personnel aspects of our agency. Leadership in clinical management is critical to our successful organizational transformation. Lake Prince Woods at Home has a focus on the total team engagement to demonstrate the value of highly experienced Clinical Manager in guiding the case management model, and the positive impact on clinical outcomes and the patient experience. Key Responsibilities Home health and Hospice care Clinical Manager oversees clinical teams for home health and hospice serving patients at home. The Clinical Manager will assist in interviewing and hiring of employees and manage nurses and other health care staff who provide services to patients in home or non-clinical care environments. Adhere to guidelines on quality of care, understanding the Conditions of Participation and Medicare/CHAP requirements. As well as assisting in the development and management of guidelines based on state and organizational standards. Chart review for hospice, HIS understanding and Oasis Understanding to be able to guide and support team members for accurate documentation. Necessary Skills Because they oversee nursing staff, organize care plans, and arrange or provide training for caregivers, home health and hospice Clinical Manager need then following skills: Personnel management, staff training and effective adult learning strategies. Be familiar with local, state and federal regulations governing home health and hospice care provision. At least 1 year in home health and hospice The most successful Clinical Manager has been a successful Case Manager for home health or hospice for at least 1 year. This will allow the further development to speak from experience and with credibility when managing productivity, requiring POC documentation in the home, case load management, and time management. Degree and Education Requirements A Bachelor’s degree preferred Associates In Nursing Required .
