Help educate the community in the home setting to manage their disease process at home, thus reducing hospitalization . Administer and educated families and patients with IV administration, Educate and perform wound care, medication management and education, disease process education. Supervisory visits with CHHA (Home Health Aides) and LVN's. Compensation is paid per visit at a per diem rate 50-90 per visit depending on type of visit.
