Licensed Vocational Nurse (LVN) Job Description
Service Areas: Los Angeles County, Riverside County (Palm Springs Included), San Bernardino County, Orange County and Ventura county. Registered Nurse (Per Diem) - Responsible for Admission and Initial Plan of Care - Supervise LVN and Home Health Aide - Good communication and coordination skills with the Hospice Medical Director and Inter-disciplinary staff regarding the plan of care, change in patient's condition, medication and DME needs. - Timely submission of documentation - Current California License as a Registered Nurse with 1 year of Hospice experience required. LVN (Per Diem) - Implements current nursing practice following the patient assessment and an approved plan of care initiated by the Registered Nurse. - Assess and provide patient and family/caregiver education and information pertinent to diagnoses and plan of care. - Provides appropriate assessment and evaluates patients' response to treatments and current medications. - Timely submission of documentation - Current California License as a LVN with 1 year of Hospice experience required. CHHA (Per Diem) - Provide personal care to patients in their own home environment on a weekly basis - Good coordination with Case Manager regarding change of condition of patient - Certification required and must have at least 1 year experience in hospice RN, LVN, CHHA requires the following: Professional license in California and clinical school diploma, current CA drivers license, valid social security card or passport, professional liability insurance if possible, CPR, current physical/TB results (within recent 6 months) and flu vaccine record, auto insurance and resume. Assessing and evaluating patient status, writing comprehensive and submitting accurate clinical notes via electronic charting. Communicating with office staff/MD via email or phone.
