Caregiver Job Description
Responsibilities and duties include, but are not limited to:
• Perform all direct consumer care and supervision requirements in compliance with all applicable governmental and non-governmental regulations and standards (i.e. Title 22 and Title 17).
• Have primary responsibility for carrying our required consumers programming, training, and services in accordance with all components of the program.
• Have primary responsibility for implementing and documenting the consumers Individualized Service Plan in compliance with established agency requirements.
• Fill out the consumer's daily task sheet and input that information to the computer system.
• Direct and assist in the maintenance and cleaning of the physical plants and grounds.
• Attend required facility staff and agency in-service trainings
• Establish and maintain appropriate and professionals’ relationships with peers, consultants, consumers, management professionals, and the community.
• Utilize supervision appropriately.
• Provide timely and appropriate written and verbal reports to the Supervisor regarding any relevant and significant issues, concerns, needs, or problems on consumer care, facility maintenance, and program services.
• Participate in interdisciplinary Team meetings in developing consumer Individual Service Plan (ISP).
• Comply with all established policies and procedures.
• It may be asked to use own vehicle for transportation of consumers. Must maintain a valid driver’s license and automobile insurance coverage in limits that meets or exceeds Company standards.
• Other tasks and duties as assigned from time to time.
