Licensed Vocational Nurse (LVN) Job Description
BASIC FUNCTIONS: Delivers care to consumers utilizing the nursing process of evaluation, planning, intervention, and implementation; and effectively interacts with consumers, families and other ID team members while maintaining standards of professional nursing.
DUTIES AND RESPONSIBILITIES:
• Utilizes the nursing process by demonstrating basic nursing techniques and procedures.
• Closely observes consumers for changes in physical and mental condition and accurately reports to program manager.
• Ensures all treatment and care of individuals conform to established SMSC Medical Procedures.
• Sets priorities for nursing action in a logical sequence according to consumers' needs.
• Evaluates a given situation and makes decisions in consultation with the Program Manager and Health & Safety Manager within the scope of licensed practice.
• Documents accurate, legible & timely ongoing evaluation of consumer status.
• Assures that up-to-date consumer medical information is passed on to appropriate Program Manager and program staff.
• Documents all communication with MD's, families, case managers program personal, etc. in consumer’s chart.
• Documents and notifies changes in consumer's condition to all appropriate parties.
• Participates in consumer/staff education regarding health information.
• Observes and documents consumer's response to pertinent medications.
• Demonstrates necessary skills and knowledge to provide care for consumers' according to their needs.
• Demonstrates ability to handle emergency situations in consultation with Program Manager and Health & Safety Manager in a prompt, precise and professional manner.
• Provides personal consumer care to promote comfort and well-being of the consumer, acknowledging physiological and psychological needs.
• Communicates pertinent information to the appropriate person in a timely manner.
• Promotes effective communication with all staff members.
• Effective & positive communicator.
• Consistently demonstrates ability to respond to changing situation in a flexible manner assessing current needs, such as reprioritizing work as necessary.
• Assists consumers, family members and staff with concern and empathy, respects their dignity and privacy, and communicates with them in a courteous and respectful manner.
• Demonstrates sound judgment by taking appropriate actions regarding questionable findings and concerns.
• Deliver medication and health care training to all Program staff members.
KNOWLEDGE, SKILLS AND ABILITIES:
• Demonstrates knowledge and understanding of all departmental policies and procedures and ability to reference from appropriate books and manuals.
• Ensures that the department is operated in an efficient and cost-effective manner.
• Adheres to Agency attendance policies.
• Demonstrates knowledge and understanding for all agencies, OSHA and CAL/OSHA safety guidelines and regulations when performing job duties related to but not limited to blood borne pathogens, hazard communications, needle sticks and other possible exposure incidents.
• Must maintain confidentiality of consumer PHI (Protected Health Information) used for treatment, payment, health care, etc.
• Good computer skills required with knowledge of Word and Excel Essential
PHYSICAL DEMANDS/REQUIREMENTS:
Physical Demands: While performing the duties of the job, incumbent is regularly required to stand, walk, use hands and fingers to handle or feel objects, tools or controls; reach with hands and arms; occasionally is required to climb or balance; frequently bends, twists, stoops and kneels; and will assist with lifting and transferring adults from wheelchairs to a variety of locations within the home and the community. Will occasionally lift household items up to 30 pounds.
