Certified Nursing Assistant (CNA) Job Description
POSITION SUMMARY: The Hospice Aide is a member of the interdisciplinary team who works under the supervision of the registered nurse case manager and performs various services for patients to meet the personal needs and to promote comfort. The hospice aide is responsible for caring for the patient and observing and reporting these observations. ESSENTIAL JOB FUNCTIONS: 1. Performs personal care needs for assigned patients in accordance with the plan of care and frequency of visits designated by the RN Case Manager. 2. Provides patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meets safety needs of patient. 4. Practices appropriate and safe use of medical devices. 5. Provides activities of daily living to include bathing, dressing, and grooming. 6. Assists in feeding the patient as indicated on the plan of care. 7. Records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure as ordered. 8. Provides skin breakdown prevention. 9. Assesses and reports on patient’s condition including significant changes to the Case Manager. 10. Calls patient and / or family 24-hours prior to scheduled visit to confirm visit. 11. Communicates to patient and / or family if there are any changes in their regular scheduled visits. 12. Assists in ambulation and exercise as instructed by the hospice nurse and physical therapist. 13. Provide normal, active, or passive range of motion exercise as indicated in the plan of care. 14. Practices safe patient transfers and positioning. 15. Assists patients with self-administration of medications as allowed by state regulations, such as “reminders”. 16. Offers and assists with bedpans and urinals. 17. Offer fluids at each visit if patient has fluid intake. 18. Adheres to agency Standards of Conduct. 19. Participates in quality assessment performance improvement teams and activities. 20. Provides safety and emergency preparedness protocols with each visit to the patient and / or family. 21. Recommends changes for the patients plan of care to the Case Managers. 22. Attends interdisciplinary team meetings no less than once a month. 23. Creates continuity of care through scheduled visits. 24. Practices collaboration of care with skilled nursing facilities. 25. Maintains eight hours of daily availability to Apex office. 26. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies. 27. Other job duties as assigned by manager.
