Location: 2013 Midwest Road Oakbrook, IL Job Type: Full-Time (FT) & Part-Time (PT) Hours: 7am-7pm, 7am-3pm, 3pm-11pm, 11pm-7am (Every other weekend required) Pay: (DOE) Start Date: ASAP Responsibilities: • Assure quality care by adhering to skilled rehab standards • Transcribe Physician orders and document all resident information as necessary • Review medication orders for completeness of information and accuracy • Provide direct skilled nursing care in the skilled rehab setting • Work collaboratively with team to help meet positive client care outcomes, and participate in care plan conferences as needed • Monitor patient’s condition and notify appropriate personnel of any changes • Assure proper maintenance of clinical records in compliance with local, state and federal laws • Maintain a safe and sanitary work environment • Respect resident’s rights and privacy • Comply with State, Federal, Local, and JACHO standards • Establish positive rapport and ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public Qualifications: • Minimum age of 18 years old • LPN or RN license in state of IL • 1 year experience working in a skilled nursing facility preferred • 1 year experience working with dementia/Alzheimer's residents preferred • Must be able to pass criminal background check and drug test
