Home Health Nurse

Clinician Home Health is in immediate preparation of providing patients quality care in the Orange County area and is currently seeking RNs and LVNs for per-visit patient care. Responsibilities: - Participate in planning, coordinating, and carrying out the plan of care for each assigned client - Accurately report and document patient’s condition, care provided, and patient/caregiver response - Assist the client with ADL’s, medications, and procedures while instructing on appropriate self-care techniques - Participate in team case conferences and communication with other nurses, therapists, doctors, and agency personnel as needed Assignments are on an intermittent basis where you will follow-up on a patient one-on-one to assess and monitor their health, safety, and independent living. As a home health nurse, you have the freedom to choose your assignment and to enjoy scheduling visits at your convenience.
