***$1,000.00 SIGN-ON BONUS*** We are searching for a full time Licensed Practical Nurse to join our team. You will coordinate, implement, and evaluate home care services for our patients. ROLE SUMMARY The Licensed Practical Nurse (LPN) builds from the resources of the organization & community to provide services to meet the needs of individuals and families within their homes. Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties. Requirements Education: Graduate of an accredited school of nursing with current licensure to practice as a Licensed Practical Nurse in the State of Oregon. Experience: Minimum of 1-year recent experience in a clinical setting; home health/ hospice care experience preferred. Licenses/Certificates/Registration: Oregon State Licensed Practical Nurse licensure required. American Heart Association BLS required. Current automobile insurance required. Valid Oregon driver’s license and reliable transportation required. Additional: Bilingual desired Additional requirements: Travel required throughout Partners In Care’s central Oregon area of service coverage including LaPine, Crescent, Christmas Valley, Prineville, Madras, Sisters.
