$2000.00 Sign on Bonus We are searching for a Full time Support RN for Home Health/Hospice to join our team. You will coordinate, implement, and evaluate hospice and home care services for our patients. ROLE SUMMARY Home Health/Hospice Support RN completes an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs. Provides a complete physical assessment and history of previous illness (es) and uses the health assessment data to determine appropriate diagnosis and need for other disciplines and resources. Develops an individualized care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Makes necessary revisions to the care plan as patient status and needs change. Completes and updates the ongoing assessments, and care plan as patient’s status changes. Provides health care instructions and education to the patient and or family as appropriate per assessment plan and patients’ caregivers’ ability to learn. Adapts teaching styles to meet patient’s needs and family’s needs. Requirements Education: Graduate of an accredited school of nursing and licensed in the State of Oregon. Experience: 1-2 years of recent medical/surgical care experience in an institutional setting. Ideal candidates will possess 1-2 years of home health or hospice experience. Licenses/Certificates/Registration: Oregon State Registered Nurse licensure required. American Heart Association BLS required. Current automobile insurance required. Valid Oregon driver’s license and reliable transportation required.
