Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans.
Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
Assist with diagnostic evaluations such as specimen collection, laboratory testing, and medical imaging procedures.
Manage complex care needs for patients with acute or chronic conditions, including pain management, and infectious disease control.
Support patients with mobility needs and assist with activities of daily living in skilled nursing or home care settings.
Document all patient interactions thoroughly using Electronic Health Records (EHR) systems such as Epic or Cerner while maintaining strict compliance with HIPAA regulations.
Collaborate with interdisciplinary teams on discharge planning, case management, and client communication to ensure seamless continuity of care.
Skills
Proven experience working with diverse patient populations across settings such as long-term care facilities, hospitals (including ICU and trauma centers), outpatient clinics, or home health environments.
Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
Excellent patient assessment skills with the ability to recognize acute pain issues and respond swiftly to urgent situations such as airway management or emergency triage.
Experience working with individuals with disabilities or developmental delays; familiarity with dementia care including Alzheimer's and memory support strategies.
Ability to communicate effectively with patients of all ages.
Knowledge of infection control practices and long-term care regulations to ensure safety and compliance at all times.
