Licensed Practical Nurse (LPN) Job Description

We are looking for a Licensed Practical Nurse to join our team and provide home health care services to pediatric patients. The successful candidate will be responsible for providing direct patient care, administering medications, and monitoring vital signs. The ideal candidate will have excellent communication skills, a strong attention to detail, and the ability to work independently. Our organization is dedicated to providing the highest quality of care to our patients, so we are looking for an individual who is passionate about making a difference in the lives of others.
Responsibilities:
Provide patient care in a home health setting.
Perform assessments of patients, chart daily and report abnormal findings to the physician.
Administer medications and treatments as prescribed by the physician.
Provide patient education on disease prevention and health promotion.
Assist with patient mobility and activities of daily living.
Maintain accurate and detailed records of patient care, condition, progress, and concerns.
Provide emotional support to patients and their families.
Administer breathing TX's and Manage Ventilator for pediatric patient that is a yr. old.
Trach suctioning, Ventilator/oxygen management, charting med Admin and vent log
Be comfortable with GJ tube care, administer tube feeds and medications daily
