Licensed Practical Nurse (LPN) Job Description
Full Time. Monday through Friday. 8:30 am - 5:00 pm Responsibilities: 1. Provide in-office Triage services Saturday and Sunday from 8:30 AM- 5PM 2. Process patient referrals and facilitate intake of patients on Mondays. 3. Appropriately handle calls either by phone or with home or facility visit when necessary or requested. 4. Provide education and emotional support for patients and families. 5. Contact visiting on-call nurses to make visits to home or facility visit when necessary or requested. 6. Accept and process new referrals that come in during work shifts. 7. Leave report per agency guidelines at the end of the shift ends. 8. Document pertinent calls and visits in accord with agency policy and guidelines. 9. Attend interdisciplinary team conference when possible. 10. Attend staff and nursing department meetings and in-services when appropriate. 11. Keep supplies readily available during shift – replenish as needed. 12. Notify staff of any deaths occurring during shift via report and email and follow death-in-home/facility procedure. Ensure that Counselor-On-Call is aware of all deaths per GSH protocol. 13. Fulfill other responsibilities as designated by supervisor, including after-hours admissions. Qualifications: 1. Graduate from an accredited school of nursing, 2. Currently licensed to practice nursing by the Virginia State Board of Nursing, 3. At least two years’ experience in acute care, home health, hospice, med/surg, or geriatric nursing, 4. Emotionally mature and physically capable of working with terminally ill patients and their families, 5. Excellent verbal and written communication, telephone, interpersonal, networking, and collaboration skills and the ability to work closely with team members, 6. Able to function independently without direct supervision, 7. Experience with electronic medical records and good computer skills, 8. Current driver’s license, valid auto liability insurance, and driving record that meets the standards set forth in current policies and procedures.
