Registered Nurse (RN) Job Description
Position is based out of our Christiansburg, VA location. Full-time, Monday through Friday, 8:30 am to 5:00 pm Responsibilities: 1. Function as an educator, facilitator and ready resource for hospitalists, discharge planners and other referral sources within the hospital on hospice-related topics such as the referral process, eligibility criteria, symptom management, and reimbursement. 2. Serve as a marketing presence for GSH, listening for feedback and communicating about referrals. 3. Work with the admissions and community relations (CR) teams in census development and educational initiatives and strategies, considering data and trends within hospitals. 4. Initiate and maintain contact with discharge planners and other referral sources for the purpose of enhancing access to hospice care and building effective working relationships. 5. Conduct information and sign-up visits and document in hospice EMR. 6. Conduct admissions on an as needed basis. 7. Assess for hospice eligibility. 8. Engage with patient and family to develop rapport and a sense of trust. Ensure that persons referred to GSH from the hospital receive prompt attention and that their expectations of hospice are communicated to the admissions team. 9. Facilitate communication and exchange of information with hospital referral sources, contract service providers and GSH so that the intake process is efficient and user-friendly. Such information includes but is not limited to patient demographics, meds, H&P, and physician orders. 10. Assess for general inpatient (GIP) eligibility. Perform GSH admission assessment for GIP patients as requested. Make visits to patients admitted to GSH under GIP care as appropriate and in coordination with the nursing department to ensure continuity of the hospice plan of care and optimum communication with hospital physicians and staff. 12. Serve as resource to RN case managers for hospice patients admitted from home/facility to hospital for GIP care. 13. Complete all electronic documentation in a timely manner and in accord with hospice policies and procedures. 14. Communicate with GSH Medical Director and/or Nurse Practitioner as needed. 15. Educate GSH staff regarding hospital guidelines and protocols. 17. Assess need and schedule in-services/educational programs for various departments within the hospital. Keep providers informed of services offered through GSH. 18. Participate in hospice and hospital-sponsored events, educational initiatives, health fairs, etc. Coordinate GSH exhibit presence at appropriate hospital-related functions. 19. Work with hospital staff to establish palliative care and hospice protocols within the hospital. 20. Establish and nurture ties with key stakeholders in the healthcare community 21. Work closely with CR department by attending CR meetings as appropriate and other activities in alignment with customer service and census development responsibilities of the position. 22. Submit weekly liaison reports to supervisor with copy to CR dept. Report may include but is not limited to statistical and other program measuring outcomes, including number of referrals and assessments, patient outcomes and satisfaction, educational programs, and related activities. Qualifications: 1. Graduate from an accredited school of nursing. 2. Currently licensed to practice nursing by the Virginia State Board of Nursing. 3. Three years experience in healthcare, highly preferred in hospice or end-of-life care. 4. Working knowledge of sales, marketing and public relations concepts, along with a knowledge of healthcare and community resources 5. Excellent communication and customer service skills. 6. Good computer skills and electronic documentation. 9. Demonstrate clinical competency, ability to interact with physicians, discharge planners, nurses, hospital staff, etc. and educate on the concept of hospice care. 10. Current Virginia driver’s license and auto liability insurance.
