Licensed Vocational Nurse (LVN) Job Description
Visiting Location Area: Los Angeles, Santa Clarita, Palmdale, Lancaster POSITION SUMMARY: Under the direction of a Director of Nursing/Registered Nurse, the Licensed Vocational/Practical Nurse provides skilled nursing care for assigned patients. He/she works cooperatively with the other members of the Interdisciplinary Team. The LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, provides counseling and supportive care to the patient and family, and participates in the care plan development and implementation. The ideal LVN must have clinical experience in implementing plans of care for patients in their homes and preferably with basic nursing documentation/charting skills. REPORTS TO: DON/Registered Nurse Minimum Requirements: Proficiency in computers is a must. Knowledge of DataSoftLogic software is preferred (but not required). Nursing license - California CPR certification State ID or driver's license. Proof of authorization to work, if applicable. Can lift and transfer patients independently. Has access to reliable transportation with an acceptable driving record. Car insurance - California Others: TB test. Physicals. Titers- MMR/Varicella. Immunization. Proof of Covid Vaccine Job Types: Full-time, Part-time Pay: $40.00 - $45.00 per visit
