LVN Field Nurse

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team. As an LVN with Sunlit , you will provide high-quality and personalized care to patients in the comfort and privacy of their own homes. Responsibilities: Conduct comprehensive assessments of patients' physical, emotional, and social needs and develop individualized care plans Administer medications, wound care, and other treatments as ordered by the physician and according to nursing practice standards Monitor patients' conditions, document changes in their health status, and communicate with the physician and interdisciplinary team as needed Provide patient education and support to promote self-care and disease management Qualifications: Current California LVN license Minimum of 1-year experience in a healthcare setting, preferably in home health/ hospice or a related field Excellent clinical, communication, and organizational skills Ability to work independently and as part of a team We look forward to hearing from you!
