Licensed Vocational Nurse (LVN) Job Description
Position Summary Under the general supervision of the RN, the LVN shall be responsible for providing training for staff, coordinate medical care for consumers, and assist with the implementation of Care Plans and doctor’s orders for consumers, various other duties as listed below. Job Responsibilities: • Coordinate medical care for consumers. o Provide hands-on care for illnesses, injuries, and emergencies involving consumers. o Act as a liaison between consumer’s regular physicians and hospital personnel. o Oversee consumer medical care in accordance with Title 22. o Collaborate with the RN to modify nursing plans as needed to accommodate the client’s daily needs. o Coordinate with the Care Provider to control communicable disease and infection. • Provide training for direct care staff. o Ensure required direct care staff training is provided on an ongoing basis. o Provide training to staff on specialized procedures. o Train direct care staff on the proper method for dispensing and giving medications to consumers. o Observe and approve direct care staff for medication certification. o Monitor staff’s techniques in dispensing medication to ensure compliance and adherence to procedures and protocols. o Provide on an ongoing feedback to the QMRP, Residential Director, Nursing staff on the training outcomes. • Assist with the implementation of care plans and doctor’s orders for consumers. o Maintain medication, treatment, and other required record keeping medical forms for each consumer. o Arrange scheduling for labs and follow-up on abnormalities and findings as required. o Document and update regularly changes in the clients’ medical condition in the clients’ chart using the proper documentation forms. (Progress Notes, etc.) o Communicate on an ongoing basis with the RN, Nursing Supervisor, QMRP, and the Medical Director of issues with consumer’s health. o Coordinate with the CNA to schedule routine client medical appointments. o Communicate with client’s parents, guardians, and/or conservators providing updates, statuses, and reassurance on any changes in client care. o Participate as a member of the Interdisciplinary Team. o Participate in Human Rights and Infection Control Committee meetings. o Pass medication for clients’ in any and all programs, as necessary. o Coordinate doctor’s rounds for assigned houses. o Continually assess for areas of improvement and develop suggestions accordingly. • Provide medical and medication supplies to assigned houses and programs. • Complete certain assigned duties, forms, and reports using a computer, as necessary. • Act as an active and involved member in TLC’s Safety Program. • Various other duties as required or needed by TLC management or Nursing Supervisor. Qualifications: • Must have a current LVN license issued by the State of California. • One year’s experience in providing services to the developmentally disabled or providing services in a nursing facility. • Demonstrate strong interpersonal, teamwork skills, leadership abilities, and a willingness to work harmoniously with different groups. • Must have patience, tact, and a pleasant professional demeanor when communicating with clients, staff and family members. • Ability to set priorities and organize work responsibilities to ensure completion of assigned tasks within preset timeframes. • Proficient in computer operations to include the following software: Microsoft Office (Word, Excel, Access). • Previous experience training and facilitating programs for staff. • Ability to chart consistently and legibly. • Ability to communicate and work effectively with people of diverse cultures, education, social, and economic backgrounds. • Ability to give direction to staff, in a polite and professional manner. • Ability to work in a team centered environment and work independently. • Maintain a valid CA driver’s license with a driving record that is acceptable for TLC’s insurance carrier.
