Licensed Vocational Nurse (LVN) Job Description
We are a Licensed Home Health Agency seeking a full-time Quality Assurance Nurse for our office. Must be a Licensed Vocation Nurse. Must have 1 years Home Health experience, or equivalent. Prefer experience in MEDICARE Home Health Agency. QUALIFICATIONS Is a Licensed Vocational Nurse LVN in the State of California or higher. LVN has at least three (1) years of experience in a home health organization, primary care clinic or a health facility, and at least one of healthcare quality management.. LVN must be familiar with Medicare, Medi-Cal and other third-party payer guidelines and regulations. LVN has excellent judgment and knowledge of current nursing practices, with demonstrated exemplary clinical, analytical and documentation skills. LVN must have good computer skills LVN is fluent in English with great communication skills LVN is self-directed and well organized LVN has current CPR Certification. LVN has current and satisfactory report on pre-employment physical examination including Mantoux TB Test or chest X-ray as required by Agency policies and procedures. POSITION SUMMARY The QA LVN Nurse is responsible for the coordination of quality management activities including evaluating and implementing goals and objectives for performance improvement to promote standards of quality. ESSENTIAL DUTIES AND RESPONSIBILITIES The following is a representation of the major duties and responsibilities of this position. Performs chart and note reviews daily, upon recertification, at discharge, and as required. Assists the QM Director with monitoring Quality Indicators, analyzing data and developing/implementing corrective measures and/or protocol changes. Assists with the preparation and maintenance of Quality Management data and reports. Facilitates monthly QA meetings and participates in Quarterly Clinical Record Reviews and QM Committee Meetings. Provides advice and counsel to staff on Quality Management related matters. Assists in the Organization's compliance with federal and state regulations Participates in the Agency's Performance Improvement Process. Activities include but are not limited to: quality indicator monitoring, Agency priorities, Agency evaluation, OASIS, orientation, staff development and education, and the Safety Program. Conforms to all agency policies and procedures.
