Registered Nurse (RN) Case Manager

Annex Healthcare Providers is looking for Registered Nurse (RN) in the San Fernando Valley, Palmdale, and surrounding areas. Annex Healthcare nurses provide direct, one-on-one medical care and make a difference in the lives of their patients and families. Job Duties: · Visit patients in their homes for one-on-one visits (up to 2 hours) · Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients · Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review · Coordination of care · Maintain client confidentiality · Educate family members on patient clinical care to enhance positive outcomes Qualifications: Active Registered Nurse license Current CPR certification Graduated from an accredited Diploma, Associate, Bachelor’s School of Nursing, or vocational nursing Ability to work a flexible schedule 1-year experience (preferred) Excellent interpersonal skills Valid driver’s license or reliable transportation We believe the healing process is often quicker in the comfort of one’s home and that patients feel more at ease and secure when surrounded by loved ones in a familiar setting.
