Full Job Description Annex Healthcare Providers is looking for Licensed Vocational Nurses (LVN) in the San Fernando Valley, Palmdale, and surrounding areas. Annex Healthcare nurses provide direct, one-on-one Home Health care /medical care and make a difference in the lives of their patients and families. The salary range is $26 - $35 hourly based on skills and work experience Job Duties: Under the direction of the Registered Nurse (RN Assist in identifying the physical and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations Conduct ongoing patient care, assessments, and documentation Administration of prescribed medication Coordination of care Provide and maintain a safe environment for the patient, including an optimum physical and emotional environment Maintain client confidentiality Educate family members on patient clinical care to enhance positive outcomes Qualifications: Active Licensed Vocational Nurses (LVNs) license Current CPR certification Ability to work a flexible schedule 1-year experience as Licensed Vocational Nurse (LVN) in Home Healthcare (preferred) Excellent interpersonal skills Valid driver’s license or reliable transportation We believe the healing process is often quicker in the comfort of one’s home and that patients feel more at ease and secure when surrounded by loved ones in a familiar setting.
