Home Health Intake RN

Cornerstone Home Health Services is seeking a caring individual to join our team. An RN who will meet patients in their homes for initial intake and evaluation assessments, additionally completing OASIS recertifications and skilled nursing visitations. Someone capable of teaching and training, as well supervising home health aides. Comfort with proper charting and communications with the office and updating on patient status, as needed. This position will require the ability to use moderate (30-50lbs) physical exertion on a regular basis. Breakdown of pay scale 3-8 visits per week $55 per visit of skilled nursing $110 for OASIS recertification $150 for SOC OASIS Our mission is to treat every person with dignity and respect, to offer the highest quality of service to all.
