Home Health LPN

Perform skilled head to toe assessment of home health patients with ability to document findings clearly. Must be able to paint a clear picture of the medical necessity within nurse's note computer charting. Experience with wound care required. Medication teaching, disease process teaching with focus on prevention of hospitalization. Requires excellent assessment, critical thinking and documentation skills. Must be able to communicate changes in patient condition to both RN case manager and physician. Must attend case conference every Monday virtually or in presence.
