Registered Nurse (RN) Job Description
We are seeking experienced, Florida-licensed Registered Nurses to join our home health clinical team. As an RN with Alignment Healthcare Services, LLC, you will provide skilled nursing services to clients in their homes, oversee individualized plans of care, and serve as the clinical lead for our CNA and HHA staff in the field.

Key Responsibilities
- Conduct initial and ongoing skilled nursing assessments for home health clients
- Develop, implement, and update individualized plans of care in accordance with physician orders
- Perform skilled nursing procedures including wound care, IV therapy, catheter care, and injections
- Administer medications and provide medication management and education to clients and families
- Monitor chronic conditions and provide disease management education (diabetes, CHF, COPD, hypertension, etc.)
- Supervise and provide clinical oversight to CNAs and HHAs assigned to shared clients
- Communicate with physicians, therapists, and interdisciplinary team members regarding client status
- Identify and respond to changes in client condition; initiate emergency protocols as needed
- Complete OASIS assessments and all required clinical documentation accurately and timely
- Ensure compliance with AHCA regulations, Medicare/Medicaid standards, and agency policies
Minimum Qualifications
- Current, active RN licensure in the State of Florida (required)
- Minimum 2 years of clinical nursing experience; home health experience strongly preferred
- OASIS certification or willingness to complete upon hire
- Level 2 background screening clearance (AHCA-compliant)
- Current CPR/BLS certification
- Valid Florida driver's license and reliable transportation
- Proof of current auto insurance
- Negative TB test or chest X-ray within the past 12 months
- Ability to pass a pre-engagement drug screening
- Proficiency with electronic health records (EHR) and clinical documentation systems
