Registered Nurse (RN) Job Description
What you'll be doing: As the RN case manager, you will plan, organize and direct hospice care for your patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. You will be the central point for the patients care plan and will help lead and mentor our Hospice Aides. Our case managers will provide care through our service area and average a case load of 12 patients. Qualifications: A passion for hospice care and the ability to build relationships with both patients and their family members Current licensure and CPR certification. The ability to work efficiently with little direct supervision. A valid driver's license and an insured automobile that is in good working order. Acute care experience in an institutional setting or home care experience is preferred, but not required. We're excited to train new RN's that are passionate about hospice care
