Registered Nurse (RN) Job Description

Our Skilled Nursing Home Health Agency is in need of a Registered Nurse (RN) who will provide nursing care in accordance with patients plan of care per physician's order. Responsibilities:
Administer medications, provide vaccinations and render treatment to client at their home of residents
Collaborate with physicians to create a Plan of Care for each patient
Consider the age of the patient and his or her emotional stability when determining appropriate treatment
Create and contribute to patients’ medical records as needed
Respond to inquiries, concerns and complaints from patients and their parents or guardians
Track and maintain medical supply inventory along with other staff members
Counsel patients and family members before, during and after treatment
Evaluates the effectiveness of nursing service to the patient and family on an ongoing basis.
Performs admission, transfer, re-certification, resumption of care and discharge OASIS for the home care patient.
Prepares and presents patient's record to the Clinical Record Review Committee as indicated.
Consults with the attending physician concerning alterations of Patient Care Plans, Check with the appropriate supervisor and make changes as appropriate and coordinates patient services.
Submit clinical notes no less than weekly, and progress notes and other clinical record forms outlining the services rendered as indicated. submit a tally of patient care visits made each day.
Participates in case conferences, discusses with the supervisor problems concerning the patients and how they may best be handled.
Discusses with the appropriate supervisor the need for the involvement of other members of the health Team such as the HHA/CNA, physical therapist, Speech therapist, Occupational therapist, Medical Social Workers, and LVN/LPN.
Inform the attending physician and other personnel of changes in the patient's condition and needs.
Participates in the patient's discharge planning process.
Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
Participates in staff development meetings and continually strives to improve his/her nursing care skills by attending in-service education, formal education, attendance workshops, and conferences, active participation in the professional and related organizations, and individual research and reading. And supervise other personnel.
Willingness to work a flexible schedule and to fill in when needed
Excellent bedside manner and communication skills
Ability to explain treatments and conditions to clients of all ages and their families.
Experience in a community/home health agency or in a hospital setting is preferred.
Must have a working knowledge of home health care and the principles and techniques of professional nursing and the required documentation that pertains to it.
Should be skillful in organization and in the principles of time management and have knowledge of management processes.
Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
Requirement:
Must pass a criminal background check, drug screening, and fingerprinting
Valid State of Alaska Nursing License and CPR certification
Degree: Associate Degree or Bachelor's in Nursing
Required experience: 2 years (Home Health experience preferably).
Must have a valid driver's license, auto insurance, and immunizations record is required.
Covid vaccine or have an exemption and willing to test weekly.
