Performs direct resident care including the provision of personal care, assistance with activities of daily living and the documentation and reporting of those activities. Assists with or provides, bath, toileting, dressing, hair care, oral hygiene, nail and skin care or any other activities of daily living to residents. Provides range of motion exercises, turns and positions residents in bed, assists residents in walking for therapeutic purposes, lifts and transfers residents using proper body mechanics, gait belt or mechanical lift to effectively and safely transfer residents where lifting, pushing, and pulling activities are involved. Adheres to and follows individual resident care plans. Distributes and collects food trays on the nursing unit and assists residents unable to feed themselves. After meals, assures that floors, tables, etc. are cleaned of food and liquids and that an accurate percent of food consumed is recorded. Ensures proper positioning of residents, utilizing specialized chairs and equipment to promote proper alignment, pressure reduction and safety. Checks alarms/devices for proper operation and function at beginning and end of shift with each interaction such as toileting, ambulation, etc., as well as on rounds. Performs simple nursing duties such as weights, temperature, pulse, respiration, blood pressure, enemas/suppositories; reports changes in patient's condition to supervisor. Transfers and/or ambulates residents according to their care plan. Sorts residents' personal laundry, places it in respective rooms and assists in keeping residents' rooms orderly.
