Registered Nurse (RN) Position Description FT/PT/PRN available JonSan Home Health is a locally owned and operated company in South Texas which is licensed by the State of Texas, and accredited by ACHC. Established in 2015 the organization has been trusted to provide quality care solutions to keep individuals as independent and safe as possible in the comfort of their own home. This position will provide quality care to patients located in and around the New Braunfels, Canyon Lake, Seguin, Floresville areas. Primary responsibilities include administering nursing care for patients 18+ in their home. This includes but is not limited to evaluating patients, creating plans of care, admissions, case management, and follow-up skilled nursing visits. Communication is crucial as the RN needs to coordinate with physician and staff in developing treatment based on physicians orders and the initial patient assessment. This role allows for teaching and mentorship by providing clinical pathways for patients and their families as well as training opportunities with peers. Qualifications Education: Graduate of an accredited Diploma, Baccalaureate School of Nursing. Licensure: Current Texas State license as a Registered Nurse. Experience: 2 years preferred in the home health field.
