Licensed Vocational Nurse (LVN) Job Description
Job description JonSan Home Health is a locally owned and operated company which is licensed by the State of Texas and accredited by ACHC. Established in 2015, the organization has been trusted to provide quality care solutions to keep individuals as independent and safe as possible in the comfort of their own home. JonSan is looking for LVNs with a passion to provide top-notch patient care. Our LVNs provide skilled nursing services to assigned patients under the direction of the registered nurse and according to physician orders. The LVN will also perform all functions in a professional and ethical manner and works with other members of the health care team to provide quality patient care. Primary Responsibilities: Assists the registered nurse in carrying out and adjusting the plan of care to meet the patient's needs Performs complete patient assessments during every home health visit Assists the physician and / or home health care RNs in performing specialized procedures following the physician’s plan of care to include but not limited to - assessments, medication administration, wound care, therapeutic treatments, and skilled teaching Prepares equipment and material for patient treatments, observing proper techniques as required Assists patients and caregivers in learning appropriate self - care techniques Recognizes, reports, and responds appropriately to emergency or life - threatening situations Reports changes in the patient's condition to the CM/ RN on a daily basis Recommends additional needed home health care services when required by the patient Communicates to the patient's physician and documents verbal orders in a timely manner Documents patient interventions that reflect delivery of safe and high - quality home health care on a skilled nursing note written during the patient encounter Turns in all documentation within 24 hours of the visit Documents all communications with the patient, family, physician, CM, pharmacy, other disciplines, and appropriate others as indicated on computerized communication notes Attends case conferences to promote coordination of care as scheduled Implements infection control and safety measures per agency policy Demonstrates effective written and verbal communication Complies with all agency policies and procedures. Promotes and maintains agency environment that is in compliance with federal, state, and local regulatory guidelines Performs other duties as assigned Patient care is provided in the field at patient’s homes. Hours vary based on patient needs and LVN availability. Required Qualifications: Active and unrestricted LVN license in the state of TX Current BLS certification Access to reliable transportation that will enable you to travel to patient homes / required trainings This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, documented evidence of vaccination, or a doctor’s diagnosis of disease Recommended Qualifications: Home care experience Experience working with the geriatric as well as pediatric patient population IV experience IV certification (must obtain within 60 days of hire) Experience with lab draws Wound care experience including experience with wound vacs Bilingual (Spanish / English) pref Physical Demands: Must be able to lift and transfer patients if necessary Ability to work in a fast paced, stressful environment Positive work environment is promoted for all agency employees. Position requires travel within the agency's service area. May work in patient homes under varying conditions with possible exposure to fumes, odors, blood and body fluids and infectious diseases; ability to work flexible schedule to meet patients' needs; some exposures to inclement weather; rotating call and emergency call. Requires ability to handle stressful situations with calmness and courtesy at all times Ability to maintain flexibility and adaptability to the work environment is critical.
