Licensed Practical Nurse (LPN) Job Description
ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILIES The essential duties and responsibilities described below are representative, but not necessarily all-inclusive, of the position: Assume responsibility for nursing care of residents during assigned shift Communicating with families and physicians. Making sure documentation in electronic Health record is accurate and up to date. Direct nursing care provided to all residents in accordance with State and Federal Regulation and administrative policies. Monitor and maintain accurate and complete records of nursing observation and care/ treatment provided in nursing notes. Maintain communication between nursing personnel. Counsel CNA’s CMT’s and Restorative Nursing Assistants and recommend corrective action to Director of Nursing. Follows policies and procedures as set by the Director of Nursing in all areas of duties, as outlined in the Employee Handbook and the Nursing Department manual. Identify emergency situations and imitate appropriate procedures. Monitor and maintain narcotic records, psychotropic Reports Maintenance and safety items to the Director of Maintenance, Director of Nursing and Assistant Director of Nursing, or to the MOD if on the weekend. Complete Monthly summaries in a timely manner. Identify emergency situations and initiate appropriate procedures. Observe all OSHA requirements. Performs other duties as required or assigned. JOB REQUIREMENTS Frequently speaking, smelling, bending, lifting items up to 10lbs., pushing and pulling items up 80lbs or more. Assisting residents with walking or pushing wheelchairs, organizing, consulting, planning and documenting. Occasionally walking, standing, sitting, climbing lifting items up to 25lbs. Adequate hearing and vision. Constantly understanding and compliance with instruction and policies. Constantly working in inside conditions, with chemicals and equipment. Occasionally working with fumes, odor and dust. Constantly working alone and occasionally with others.
