Licensed Vocational Nurse (LVN) Job Description
Provide trach care : maintain airway patency by removing mucus and encrusted secretions, maintain
cleanliness and prevent infection at the trach site, prevent skin excoriation and infection around
the trach incision, promote comfort, prevent displacement, monitor effectiveness of vent while
connected during the night
Assess respiratory status (ease of breathing, rate, rhythm, depth, lung sounds, and oxygen
saturation level), heart rate , and secretions from the trach site (character and amount).
Administer medication following doctors instructions, monitor for side effects. Provide set ups for
meals, keep aspiration precautions in place.
Assess Cardiac, respiratory, GI, GU, musculoskeletal and neuro status daily.
Monitor skin integrity 3x per day and report any issues to MD, reposition Q 2hrs when in bed
Provide continuous oxygen as per MD orders 31/min via trach collar, monitor for signs of
desaturation
Provide AOL's including showering, toileting, feeding, dressing. Maintain safety ,aspiration and
fall precautions in place .
Provide G-tube site care daily and PRN Monitor for s/sx of infection
Monitor BM for s/sx of diarrhea and constipation
Monitor V/S, promptly report any out of parameter findings to physician. Provide TheraVest and
Cough Assist treatments daily
