RN Resident Care Manager North Central Care & Rehabilitation seeks an RN Resident Care Manager- $10,000 sign on bonus. Position Summary Under the supervision of the Director of Nursing, the RN Resident Care Manager assumes responsibility and accountability for the supervision and direction of the overall needs of the resident, including the resident’s medical, psychological and spiritual needs. The RCM will be responsible for Admissions and Discharges to the facility with associated assessments. Forms a baseline care plan per federal guidelines and assures the completion of the 48 hour care plan review conference. Assists the nursing team in obtaining the best quality of life for the residents, guided by the resident’s expressed wishes or known wishes. Along with social services, the RCM participates in all scheduled quarterly and PRN care conferences in coordination with MDS scheduling. Assist with assessment and CAAS review, meeting at the time schedule mandated by state and federal guidelines. Oversees the nursing assessments implementations, and assists with the evaluation of the plan of care. Adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nurses and nursing assistants. The RCM will be the main team leader who motivates and oversees the floor staff. The RCM will assist with all MD orders, MD rounds, and communications with the provider. The resident care manager will lead their team to assure that all meds are reordered timely and accurately in correlation with company policy. They will communicates resident status changes with the Director of Nursing and work with other departments within the facility to meet the resident’s needs. Follows all facility policies and procedures. Performs duties as defined by the State Nurse Practice Act. Job Duties · Will be a team leader and oversee all Licensed Nurses and NAC’s to assure appropriate care and guidance. · Perform all tasks associated with the resident admission and discharge process to ensure the needs of the new resident are met per federal state guidelines. · Assists in the resident admission process to ensure the needs of the new resident are met. · Form baseline care plans and with the help of the inter-disciplinary team to meet the needs of the resident ant assure completion of the 48 hour care plan conference review. · Participates in QAPI meetings and oversees team projects. · Participates in Safety Committee · Works with the Administrative nursing team in maintaining quality care and appropriate financial reimbursement for resident care. · Reviews resident status and works closely with the primary nurse to communicate with the resident’s physician to meet the resident’s needs. · Participates in Resident Care Conferences and communicates appropriately with residents and/or the family/responsible party. · Attends resident focus meetings i.e.: Psych Meeting, Rehab, Restraint etc and participates in the process to enhance the resident’s quality of life. · Available to work in a Primary Nurse role as needs arise or other duties assigned. · Functions in a supervisory role to facilitate quality resident care, teaching and directing NAC’s in resident care. · Functions in a preceptor role with other licensed staff to ensure quality nursing care. · Reports any care concerns to the Director of Nursing. · Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post-acute care facility. · Ensures assigned work area (i.e. nurse station, med.
