Registered Nurse (RN) Job Description
Job Title: Service Supervisor Reports To: Agency Director FLSA Status: Non-Exempt Duties and Responsibilities include the following. Other duties may be assigned. Supervisory Responsibilities: Directly supervises 20 employees. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. 1. Assures excellent patient care, safe operations, high customer satisfaction and program success through operational integration of clinical, communication functions; directly managing clinical services, supervision of all in-home aides, companion, and sitter and respite staff.* 2. Completes initial assessment plan of care annually and/or as needed.* 3. Responsible for conducting supervisory visits for each in-home aide and each client’s place of residence at least every three (3 months) and annually; a quarterly supervisory visit to the home of each client. Documentation shall be maintained in the agency’s record for any and all corrective actions.* 4. Reviews client’s plan of care at intake and every 3 months. Notifies physician of any changes in client’s condition which indicate the need for altering the plan or for terminating services.* 5. Sevres’s in a clinical/administrative role with the Agency Director.* 6. Responsible for planning, coordinating, and evaluating formal quality assessment and improvement efforts through the organization using a planned, systematic and organization wide approach. Monitors quality assessment and performance improvement activities and providing coordination, consultation and assistance in pursuing quality goals.* 7. Works closely with community agencies LME-MCO and their respective systems; Medicaid and North Carolina Department of Human Services.* 8. Has a functional understanding of state and local regulations and protocol/procedures in the service area.* 9. Proposes, reviews, implements, and enforces Infinity Care’s policies and procedures.* 10. Communicates and interprets policies and procedures to staff.* 11. Reviews policies and procedures and revises as needed at least annually.* 12. Initiates and facilitates changes to improve consumer care and program performance through effective communication, collaboration and interdisciplinary problem solving.* Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills: To perform this job successfully, an individual should have knowledge of word processing software, spreadsheet software, database software, and email. Education/Experience: High school diploma or general education degree (GED); and two to four years related experience and/or training; or equivalent combination of education and experience.
