ROSS MEMORIAL HEALTH CARE CENTER MDS/CARE PLAN COORDINATOR JOB DESCRIPTION PURPOSE: Accountable for continually improving the quality of resident care by coaching, intervening and facilitating the resident assessment inventory in an interdisciplinary and timely process. Coordinates the resident admission process and ensures the data collection is though, timely and fully integrated into the resident’s care plan. Maintains regulatory and professional standards/requirements. This position serves the following resident age categories of adult and geriatric. (Ross Memorial Health Care Center discourages admission of adults below the age of 65 years old.) Qualifications: Education: Graduate of an RN Nursing Program and hold a current State of Georgia License. Baccalaureate degree in nursing preferred. Graduate of an LPN nursing program and hold a current State of Georgia License and work under the direct supervision of an RN at RMHCC. Experience: Minimum of three years of long term care clinical practice and have working knowledge of the Federal/state Regulations. Certificate in MDS preferred. Responsible to: Director of Nursing Supervises the disciplines performance of the MDS/Care Plan data in timely and accurate manner. Evaluation Criteria: Standard. Maintains a current and accurate MDS/Care Plan process. Demonstrates interdisciplinary teaching to all departments to ensure compliance with the MDS/Care Plan process. Demonstrates current knowledge of mandates and updates set forth by regulatory agents. Communicates effectively both written and oral information to ensure compliance with MDS/Care Plan process. Demonstrates accountability for the integrity of the MDS/Care Plan data. Demonstrates analytical ability through ongoing quality assurance initiatives related to the MDS/Care Plan process. Dress Code. Maintains a professional dress appearance with white lab coat and Name Tag. Physical Demands and working conditions: Exposure to some noise and stress. Ability to work and talk with people. Read, write and speak English. Ability to speak Spanish a benefit but not a requirement. Sit 60% to 70 % of the shift. Bend and stoop frequently. Lift 25 pounds or more. Sufficiently mobile to push all clinical equipment. In an emergency is expected to respond and includes the possibility of evacuation of residents to directed area. Demonstration of basic computer skills. Communicate via telephone and other automation equipment. Responsibilities and Duties: Coordinates the Resident Assessment on all residents. 1. Completes the MDS tool(s) in a timely manner 2. Maintains a current MDS tracking tool for all residents as directed by the RAI. 3. Completes/directs the completion of RAPS in a timely manner 4. Completes quarterly pain & fall assessments on residents. 5. Seeks input from all members of the healthcare team to ensure all residents information (MDS, Assessment tools, RAPS, and Care Plan) is accurate and current 6. Assess the need to implement or delete assessment tools. 7. Conduct weekly care plan conferences reflective of 24-hour care and services. 8. Develops the Care Plan in conjunction with the interdisciplinary team that is measurable and realistic designed to promote quality of life and quality of care in a comprehensive manner. 9. Directs the healthcare team members to ensure dignity, respect, individual choice is provided to each resident. 10. Makes revisions to the Care Plan based upon input from all members of the healthcare team. 11. Ensure proper notification of family/significant other and resident for their attendance in Care Plan Conference. 12. Assess safety factors for all residents during Care Plan Conference. 13. Provide education for all members of the healthcare team as needed to ensure ongoing quality of the MDS process. 14. Maintain compliance with PPS, Case Mix, OMRA, and significant change assessments. 15. Complete discharge/reentry tracking forms. Administrative team member.
